Initial Consultation 1 Hour £100
Includes
- Assessment of medical symptoms in relation to food
- Full dietary assessment/assessment of food diary
- Dietary Advice
- Written information to support dietary advice given
- Consultation report copy for you & GP
Follow up consultations 30 minutes £60 includes
- Assessment of medical symptoms in relation to food
- Review of dietary advice & symptoms
- Written dietary advice to support advice given
- Consultation report copy for you & GP
Annual Coeliac Review Consultation 1 Hour £100 includes:
- Review of symptoms associated with Coeliac Disease
- Dietary assessment
- Calcium assessment & advice
- Review of gluten free prescription
- Advice on gluten free prescriptions
- Advice on gluten free products
- Advice on other dietary aspects of coeliac disease such as lactose intolerance/healthy eating for weight reduction
- Request for coeliac bloods with GP
Fees
- Include additional time required outside the consultation to produce your written report, send copies to Gp and or Consultant
- Fees are in line with guidance from the British Dietetic Association
What happens if I cancel my appointment?
- If you need to change your appointment please contact Dawn to rearrange your appointment.
- Cancellation of your appointment within 24 hours of the appointment date & time will incur a charge of £60
Payment
- Please make payment by cash or online banking on the day of your appointment.
- If paying by online banking, please include your name as a reference
Medical Insurance Payments
Dawn is a recognised health care provider/professional for Bupa, Axa PPP & Vitality.
Before your appointment with Dawn you will need to contact your insurance provider to obtain an authorisation code and to check what you are covered for on your insurance policy.
If your policy does not cover the full consultation fees you will be asked to pay the difference at the time of your appointment.
Consent
Medical Information & Consent
- It is assumed by attending your appointment you are giving consent to have your medical details recorded.
- Dietitian Nutrition North East is registered with the data protection register to say that medical notes are stored securely.
- You will be asked to complete a medical assessment form which asks for your signature to confirm you are happy for Dawn/Dietitian Nutrition North East to inform your GP of any advice given to you.
